Imaging plays an important role in liver transplantation, from the assessment of the graft parenchyma, vasculature, and biliary system, to detection of complications and infections, to surveillance for recurrence of malignancy. In the setting of graft dysfunction, it is important to note that imaging cannot assess graft rejection; so, its role is to exclude other causes of disease. The choice of imaging modalities is largely based on the specific indication, with ultrasound often being the first-line modality, especially in the early postoperative setting. Doppler ultrasound provides important information regarding the quality and quantity of flow, which cannot be obtained from anatomic imaging such as CT and MRI. MRI and specifically MRCP sequences provide a more holistic view of the biliary tree and can help in grading and monitoring of biliary disease. In the setting of liver transplantation as treatment for an underlying malignancy, CT and MRI take a central role in the surveillance for recurrent disease.
