Social workers are an essential component of the multidisciplinary team. This comprehensive assessment includes the identification of social support networks, knowledge gaps, past or current substance use/misuse, mental health challenges, adherence, and motivation. Mitigation of potential negative factors is provided by way of education, supportive counseling, resource navigation, and psychoeducational support.
