The Prevalence and Consequences of Neck Problems in Professional Freelance Musicians in Denmark: Cross-Sectional Data

Background and Purpose: In the literature, many studies have investigated musculoskeletal problems among professional musicians. However, most of the evidence comes from classical musicians including symphony orchestra musicians, while problems among the non-classical freelance musicians are relatively understudied. This project aimed to determine the prevalence and duration of neck problems and explore associations with gender, age, primary instrument, daily playing hours, breaks during rehearsal and warm-up among professional non-classical musicians in Denmark. Further, we investigated the consequences of the neck problems in terms of cancelled playing time at home and with others, concert cancellations, and in terms of difficulties in activities at home, leisure time activities and sleep. Material and Methods: A cross-sectional questionnaire survey was conducted among full-time members of the Danish Musicians’ Union’s unemployment insurance fund, and 563 filled in the questionnaire and participated in the study. The prevalence of neck problems, gender, age, primary instrument, daily playing hours, breaks during rehearsal and warm-up was described. We used logistic regression analysis to estimate the odds ratio (OR) for occurrence and duration neck problems by gender, age, primary instrument, daily playing hours, breaks during rehearsal and warm-up. The logistic regression analysis was subsequently repeated and adjusted for gender and age. Consequences as cancelled musical activities and difficulties in non-musical activities due to neck problems were stratified by gender and tested with a χ2 test. Results: Among the participants, 97.4% were freelance musicians. Neck problems were common. Neck problems within the last 12 months were reported by 59.1% of the musicians with a significant gender difference (79.2% in females vs. 53.1% in males). An odds ratio of 3.36 confirmed the significant association between female gender and occurrence of neck problems. Also, neck problems were associated with being vocalists (musicians who use their voice as their primary instrument). Vocalists had an odds ratios of 2.33 and 4.24 for neck problems and prolonged neck problems, respectively. The odds ratio increased further to 5.49 when adjusted for age and gender, showing that the significance was not only due to most vocalists being females. Overall, very few of the respondents reported that they cancelled playing time due to pain; however, women cancelled playing at home due to neck problems more often than men. Less than 1% of the participants, independent of gender, had cancelled playing with others or playing concerts due to neck problems. Conclusion: Neck problems were common among non-classical freelance musicians, even when compared to other professions. This cross-sectional study demonstrated an association between neck problems and musicians of female gender as well as vocalists. Compared to earlier findings among classical musicians, neck problems were even more frequent among the non-classical freelance musicians. That vocalists specifically were affected was a new finding. The reported high frequencies of neck problems call for preventive intervention.
