Several reviews have enumerated the plethora of drug trials aimed at improving negative symptoms and/or cognitive performance in patients affected by schizophrenia (Sabe et al. Neurosci Biobehav Rev 144: 104979, 2023). The aim of this chapter is to highlight two of the reasons for the high rate of failures in these indications and to suggest potential remedies: (a) reduce heterogeneity of patients recruited for the trials by improving sample stratification and (b) employ monotherapy rather than add-on trial design.
