This chapter examines the status of gender parity in academic radiology across different global regions. While the representation of female first authors in relation to the regional female radiologist proportion has improved, particularly in journals from the United States and Europe, women remain underrepresented in senior authorship. Regional disparities are evident, with Asian journals showing the lowest female authorship rates and limited data available for Africa and Oceania. Barriers to parity include structural disadvantages, mainly due to unequal family care responsibilities, and persistent gender biases in leadership perceptions. Finally, targeted measures are discussed—such as mentorship, flexible work models, transparent promotion processes, and cultural shifts in leadership expectations—as strategies to achieve gender parity in radiological research worldwide.
