The chapter begins with the radiologic workup of ischemic stroke, including basic noncontrast head CT, CT angiography, and MR diffusion weighted imaging. It covers the basic, salient anatomy that a radiology resident should know with respect to the anterior and posterior circulations as well as the Circle of Willis. Further material delves into in-depth vascular, territorial, and functional anatomy, including Brodmann Areas, and stroke presentations. This information is presented in a clinical-radiologic context, describing unique syndromes, eponyms, and material that could also be useful while at the stat desk or on clinical rotations. The ischemic stroke section is concluded with a discussion of the DAWN, DEFUSE 3, and EXTEND trials and stroke perfusion imaging. Common pitfalls are discussed, such as stroke mimics and contrast staining. Venous infarcts are then reviewed with close anatomic correlation provided courtesy of licensed Radiopaedia imaging from Dr. Craig Hacking. These include cortical vein and dural venous sinus thromboses. Diagnostic utility of CT and MR venography is reviewed, as are the pitfalls present in time-of-flight MRI. Finally, the chapter ends with a review of hemorrhagic stroke etiologies to include aneurysm, AVM, RCVS, amyloid angiopathy, PRES, and ARIA.
