Anesthesia and the Heart Transplant

A 50-year-old male diagnosed with ischemic cardiomyopathy was waitlisted for a heart transplant. The preoperative evaluation was significant for pulmonary hypertension and chronic congestive heart failure with an EF of 10%. An AICD and a Impella (percutaneous pump) was placed. Pulmonary hypertension was treated with prostacyclines. Despite treatment, the patient was placed on ECMO. UNOS found a heart that was classified as a DCD donor. The case went well, but the patient could not be weaned off bypass.
