This article reports the case of a 65-year-old male patient with massive, symptomatic primary tricuspid valve regurgitation (TR) due to a structural defect of the septal leaflet following surgical tricuspid valve leaflet repair more than two decades ago. As the patient was declined for high-risk tricuspid valve surgery, the local heart team consented the patient for transcatheter tricuspid edge-to-edge repair (T-TEER). The T-TEER procedure resulted in reduction of TR to mild without significant tricuspid valve stenosis. This case highlights the fact that T-TEER can be considered as an alternative option also among patients with complex primary TR in order to avoid high-risk cardiac surgery.
