The increasing prevalence and unfavorable prognosis of mitral valve regurgitation have fueled the widespread development of transcatheter techniques for mitral valve repair and replacement. Due to the anatomical and physiological complexity of the mitral valve apparatus, a thorough understanding of its structure and function is essential to comprehend the various mechanisms responsible for valve insufficiency. This chapter examines the anatomic details of mitral valve apparatus, illustrating how alterations in each individual component are linked to specific mechanisms of mitral regurgitation. The adoption of multimodality imaging facilitates an accurate analysis of mitral regurgitation, enabling the selection of the most suitable percutaneous intervention and detailed procedural planning.
