During the last few years, transcatheter aortic valve implantation (TAVI) for the treatment of aortic stenosis has expanded its indications towards younger and lower-risk patients. Evolving options in therapeutic strategies have identified the central role of the heart team in optimizing patient selection and clinical indications. Evaluation of frailty and considerations about futility must be included in the decision-making process, together with operative risk scores and comorbidities.
