Virtual Surgical Planning in Orthognathic Surgery in Cleft Patients

Virtual Surgical Planning is an evolving means of pre-surgical planning in orthognathic surgery by predicting the movements required during surgical manipulation. Traditional surgical planning is an arduous and time intensive process that requires high accuracy to execute the surgical plan. The incorporation of virtual surgical planning has allowed craniofacial surgeons to theoretically predict and plan surgical movements in bimaxillary and mandibular osteotomies using computed topographic (CT) imaging. The intricacies of orthognathic surgery require high levels of accuracy and virtual surgical planning’s ability to control for millimetric movements in bony and soft tissue reconstruction is invaluable. The use of virtual surgical planning in planning also lends way to the use of pre-determined patient specific cutting guides which is an expanding component in orthognathic surgery planning. Traditional surgical planning (TSP) required analysis of patient photographs, 2D cephalometric radiographic analysis in conjunction with patient dental impressions and splint production. Virtual surgical planning has removed the laborious guesswork of TSP by utilizing computer-aided 3D renderings via visual comparisons of bony, soft tissue, and dental manipulation pre- and post-operatively. Virtual surgical planning does have an initial cost-burden due to its expense with comparable outcomes to TSP but virtual surgical planning has been shown to significantly reduce surgeon time planning and time spent in the operating room.
