This chapter discusses interpretation of capillaroscopic images. The landmark paper in which capillaroscopy has been evaluated for the first time in a large group of healthy controls will be described. Also, the seminal papers concerning capillaroscopic image interpretation in systemic sclerosis (SSc) will be elaborated on through pictural educational examples. Transition of primary Raynaud’s phenomenon (RP) to secondary due to SSc will be touched briefly, as this will be described in detail elsewhere (chapter “ Nailfold Capillaroscopy in Systemic Sclerosis (Including Associations) ”). Based on the above mentioned milestones, the standardisation effort based on consensus between the Scleroderma Clinical Trials Consortium (SCTC) and the European Alliance of Associations for Rheumatology (EULAR) study group on microcirculation in rheumatic diseases members will be described. In this way the capillaroscopic characteristics that ought to be described (density, dimension, morphology and haemorrhages) and their interpretation as normal or abnormal will be exemplified through capillaroscopic images. The fast track algorithm to reliably classify a capillaroscopic image as scleroderma pattern or not will be explained. Moreover, reliability of terminology used throughout the chapter will be stipulated where appropriate. Finally, as introduction to chapter “ Quantitative Analysis ” and chapter “ Nailfold Capillaroscopy in Systemic Sclerosis (Including Associations) ” “Nailfold capillaroscopy in systemic sclerosis”, qualitative image interpretation and (semi)-quantitative scoring will be described.
