Lower gastrointestinal (GI) bleeding can be challenging to diagnose and treat. The transcatheter management of lower GI bleeding, a procedure established over 60 years ago, has evolved into a cornerstone procedure within interventional radiology (IR). This chapter reviews the various causes of lower GI bleeding, including diverticulosis, angiodysplasia, neoplasms, ischemic colitis, and anticoagulation-related bleeding, with an emphasis on the role of transcatheter angiography and embolization in managing lower GI bleeding. It highlights the various steps in performing diagnostic angiography and embolization with attention to the stepwise process in progressing through this procedure. Advances in catheter technology, imaging techniques, and embolic agents have significantly improved the precision and success of these IR procedures. Modern approaches, such as microcatheter use and coil embolization, are now standard in treating lower GI hemorrhages. The chapter provides further insights regarding the evolving tools used in embolization, ranging from conventional embolic agents to recently approved embolic platforms.
