Autologous Peripheral Blood Products in Regenerative Medicine: Preparation, Mechanisms, and Applications

Autologous peripheral blood products (APBPs), such as platelet-rich plasma and platelet-rich fibrin, represent a growing class of regenerative therapies that harness the bioactive components of blood such as growth factors, cytokines, and extracellular vesicles to modulate inflammation, promote tissue repair, and support cell regeneration. These therapies have shown promise in treating musculoskeletal injuries, osteoarthritis, and ocular surface disorders. However, variability in preparation protocols, including centrifugation speed, activation methods, and the use or absence of anticoagulants, continues to limit clinical standardization and reproducibility. This chapter reviews the biological rationale, processing workflows, and clinical evidence for each APBP, drawing attention to their therapeutic mechanisms and limitations. Furthermore, this review underscores the importance of regulated practices and infection control, as unstandardized applications pose safety risks. Emerging data and technological advances may enable more targeted and effective use of these products, offering a path toward more personalized and durable regenerative treatments.
