Omentum and Gastro-Omental Free Flap

The omentum free flap was initially described by Mclean and Buncke in 1972 and later augmented to include the composite gastro-omental flap introduced by Baudet in 1979. The omentum has intrigued physicians and philosophers for centuries for its unique properties. The omentum possesses several advantages over traditional skin and muscle-bearing flaps due to its angiogenic, immunogenic, and wound healing properties, making it an excellent option for the reconstruction of complex defects and nonhealing wounds following salvage surgery. Both the omental and gastro-omental free flaps are supplied by the right and left gastroepiploic vessels but is generally harvested using the right gastroepiploic vessels due to their dominant blood supply and larger diameter.
