The temporalis flap is a versatile and reliable reconstructive option that can be used to repair soft tissue defects in various regions of the head and neck. The temporalis muscle is a fan-shaped muscle innervated by the mandibular division of the trigeminal nerve and vascularized by branches of the internal maxillary and temporal artery, giving it a robust arterial supply. It can be used in facial, maxillary, and intraoral reconstruction as well as in facial reanimation. This flap has a low rate of complications and avoids the need for a separate surgical site. Although the temporalis flap has become less frequently used due to adoption of microvascular free flaps, it remains a robust reconstructive option.
