Open Debridement, Antibiotic, and Implant Retention (DAIR) for Periprosthetic Joint Infection in Total Knee Arthroplasty

In a study, open debridement, antibiotic, and implant retention (DAIR) for periprosthetic joint infection (PJI) of primary total knee arthroplasty (TKA) was successful in 64% of “early” PJIs (implant <1 year) versus 38% of “late hematogenous” PJIs (implant >1 year). For late PJIs (implant >1 year), Staphylococcus aureus and gram-negative infections were risk factors for DAIR failure. In other study, DAIR success after PJI of primary TKA was 45% and was highest in early PJIs. Success was lower in both hematogenous and chronic infections. Some authors have found a success rate of 70% after DAIR in PJI after primary TKA. ASA (American Society of Anesthesiologists) >2, BMI (body mass index) >35, CRP (C-reactive protein) >200, and Staphylococcus aureus growth were predictors for DAIR failure.
