Complications Associated with Repair of Transposition of the Great Arteries and Associated Lesions

Transposition of the great arteries (TGA) is a congenital heart defect in which there is malalignment of the aorta and pulmonary artery where the right ventricle (RV) gives rise to the aorta and the left ventricle (LV) gives rise to the pulmonary artery. There are three main groups of TGA: (1) TGA with intact interventricular septum (TGA-IVS), (2) TGA with ventricular septal defect (TGA-VSD), and (3) TGA with VSD and pulmonary stenosis (TGA-VSD-PS). This chapter will review the operative steps of the arterial switch operation. These steps include (1) great vessel dissection/mobilization, (2) coronary artery button transfer to the neoaorta, (3) neoaortic reconstruction, and (4) neopulmonary artery reconstruction. Each of these steps is attended by potential complications. This chapter will depict the anatomic complications and the solutions thereto pertaining.
