Understanding temporal bone surgical anatomy is essential in cochlear implant (CI) surgical placement. This chapter highlights the critical anatomical landmarks relevant to cochlear implantation, varying from the standard transmastoid posterior tympanotomy approach to the subtotal petrosectomy (SP), a very useful procedure especially in complex cases. Building a proper temporal bone anatomical knowledge is mandatory for all surgeons involved in otological and cochlear implants surgery as well.
