Telemedicine in Chronic Disease Management: Transforming Care Delivery

Telemedicine has become a transformative tool in healthcare, creating significant opportunities for managing chronic diseases such as diabetes, hypertension, cardiovascular conditions, and chronic respiratory disorders. This work synthesises evidence on the benefits of telemedicine, including enhanced accessibility for remote and underserved populations, cost savings, and improved disease management through remote monitoring and virtual consultations. However, several challenges persist, including technological barriers, socioeconomic disparities in access, and limitations in virtual clinical interactions. Strategies to address these obstacles—such as improving digital literacy, leveraging innovative technologies, and implementing equitable policy frameworks—are explored. By consolidating current findings, this work underscores the importance of a balanced and inclusive approach to telemedicine in chronic disease management. The insights aim to support healthcare professionals, policymakers, and researchers in optimising the long-term potential of telemedicine while mitigating the risk of exacerbating existing health inequities.
