Autologous fat grafting has been classified as a secondary reconstructive procedure, and it is an upcoming technique for breast reconstruction and augmentation. This chapter briefly touches on patient selection and education before autologous fat grafting (AFG) of the female breast, as well as the intraoperative documentation, and illuminates the follow-up care. How to avoid complications or, if they occur, the management of these has also been discussed. The main focus is to give an overview of the common benign findings after AFG and how to find and manage suspicious lesions using ultrasound of the breast. Ultrasound can be used not only to manage AFG complications but also to monitor the persistence of the transplanted fat in the breast.
