Levothyroxine (LT4) has been the standard therapy for hypothyroidism since the 1970s. To date, LT4 is available as tables, soft gel capsules, and liquid formulations. Liquid and soft gel formulations can be considered in patients with clinical conditions that reduce gastric acidity or with intestinal diseases associated to malabsorption. These formulations may also be useful in children due to their easier way of administration, and in pregnant women with hyperemesis. Besides LT4 monotherapy, triiodothyronine-thyroxine combination therapy can be considered in selected hypothyroid patients complaining symptoms despite biochemical tests indicating adequate replacement, mainly after thyroid surgery.
