The spleen is the most commonly injured solid organ in blunt trauma and while splenectomy was previously quite common, the management of this injury has evolved. This chapter discusses the recognition and management of spleen injury, including a review of the principles regarding operative, nonoperative, and postoperative care in this patient population.
