Metabolic and bariatric surgery is the most effective treatment for severe obesity. However, recurrent weight gain occurs at varying rates depending on the initial procedure and other factors. Revisional bariatric surgery can be done safely and effectively offering patients additional options for treatment of obesity and obesity related diseases. In this chapter, we describe minimally invasive reoperative approaches to address recurrent weight gain after laparoscopic adjustable gastric banding (LAGB), sleeve gastrectomy (SG), and Roux-en-Y gastric bypass (RYGB); specifically, the conversion procedures of LAGB to SG, LAGB to RYGB, SG to RYGB, and SG to biliopancreatic diversion with duodenal switch (BPD-DS), as well as the revision of RYGB via type I distalization.
