This chapter provides insight into some of the chaos our patients, and us, as street medicine clinicians can come up against while doing street psychiatry. This chapter discusses how time is different on the street and how clinicians need to adapt their practice to “street-time.” It explores ways to consider informed consent and clinician’s competing priorities around using involuntary commitment to treat patients. This chapter also explores the profound impact of grief on homeless individuals and how grief may present as well as special considerations for clinicians around end-of-life care on the streets. It explores encampments and how “street sweeps” worsen the health and social outcomes of people surviving on the streets.
