Most mycotic femoral artery aneurysms are pseudoaneurysms and can result in secondary hemorrhage with a potential for limb loss. Direct arterial repair is not feasible in most instances. Ligation of the femoral artery without revascularization in patients with involvement of the femoral artery bifurcation results in an amputation rate of greater than 30%. The options for revascularization include the use of (a) internal iliac artery, (b) superficial femoral/popliteal vein, and (c) extra-anatomic bypass (obturator bypass/lateral bypass over the iliac crest). The common organisms causing mycotic femoral artery aneurysms are S. aureus, E. faecalis, S. epidermidis, Salmonella, E. coli, and anaerobic species.
