Currently, every cataract surgery has become a true refractive surgery (Kim, Lancet 393:2085–2098, 2019) hence increasing pressure to obtain better refractive results. Nor can we forget the increased demand for multifocality from our patients. However, we cannot always advise the implantation of intraocular multifocal lenses; personal and ophthalmological reasons exist which limit the performance of said lenses. Nevertheless, upon carrying out a complete anamnesis and a meticulous exploration of not only the anterior pole but also the posterior pole, as well as the pertinent complementary tests (corneal tomography, endothelial count, biometry, and optical coherence tomography (Optical Coherence Tomography)), we would be able to advise the most appropriate intraocular lens for our patient. In these circumstances in which we cannot implant high-technology lenses and we would recommend conventional monofocal lenses, we have the option of recommending an advanced and improved monofocal lens. The exponent of this group of lenses is the Eyhance lens by Johnson & Johnson.
