Embolization Techniques for Bone Tumors

The treatment options for spinal bone tumors include surgery and radiotherapy. Surgery, especially for highly vascular tumors, carries the risk of significant blood loss. Preoperative endovascular embolization has emerged as a valuable adjunctive procedure to reduce intraoperative bleeding, facilitate tumor resection, and improve surgical outcomes. Tumor embolization involves selectively blocking the tumor-feeding vessels using various embolic agents, guided by detailed spinal angiography (DSA). This procedure requires careful preoperative planning, knowledge of spinal vascular anatomy, including the artery of Adamkiewicz, and the selection of appropriate embolic materials (particles, liquids, coils). Optimal timing for subsequent surgery is typically within 24–72 h post-embolization. Potential complications include spinal cord ischemia, tumor swelling, stroke (especially with cervical lesions), and access site issues, necessitating a meticulous technique.
