Minimally invasive adrenalectomy is considered a safe and effective treatment option for adrenal tumors. Laparoscopic and robotic approaches provide significant advantages over open surgery. Laparoscopic adrenalectomy, introduced in 1992, has become the standard treatment for small and medium-sized adrenal lesions, with various surgical approaches available including transperitoneal and retroperitoneal routes. Robotic adrenalectomy is also gaining popularity due to its advantages over laparoscopy, such as better ergonomics and 3D imaging. In this chapter, surgical techniques for minimally invasive adrenalectomy including anterior transperitoneal, posterior retroperitoneal, lateral transperitoneal and lateral retroperitoneal approaches, their advantages and disadvantages are discussed in detail. Further advancements in robotic surgery may continue to improve outcomes in the future.
