LLDN was first pioneered in 1995 to reduce complications of open procedures and has since provided superior postoperative pain management, shorter hospital stays, and improved cosmetic results. Hand-Assisted Laparoscopic Living Donor Nephrectomy (HALLDN) and Robotic-Assisted Laparoscopic Living Donor Nephrectomy (RALLDN) are two variants that offer distinct advantages. Comparative analyses have shown that all techniques have comparable recipient outcomes, with no significant differences in graft function or survival. The selection of a donor kidney (left or right) and the consideration of anatomical variations have a significant impact on surgical decisions. The focus is on optimizing outcomes while ensuring donor safety. In this chapter, a comprehensive surgical technique for LLDN is presented, which emphasizes perioperative care, intraoperative considerations, and postoperative management. This includes early recovery protocols and long-term follow-up strategies. Although complications are rare, it is important to exercise caution to reduce the risk of bleeding and conversion to open surgery.
