The confluence of Artificial Intelligence (AI), Digital Twins, Extended Reality (XR), and the Metaverse is transforming the future of intelligent healthcare with real-time physician–patient dialogue, disease prediction via predictive analytics, and virtual therapy. This research explores how all these technologies combined enhance diagnostic precision, optimize treatment protocols, and transform patient care through data-rich, AI-fostered simulations and virtual healthcare environments. AI-powered predictive analytics allows disease and risk to be identified at an early stage, while Digital Twins allow for real-time, personalized patient models with the capacity to model treatment and use precision medicine. XR technologies like Virtual Reality (VR), Augmented Reality (AR), and Mixed Reality (MR) enhance medical training, telesurgery, and rehabilitation therapy, allowing immersive and patient-centered healthcare. Metaverse comes with decentralized virtual hospitals, AI-powered telemedicine, and blockchain-secured health records, and delivers secure, scalable, and globally accessible healthcare services. With such enormous potential, data privacy, ethics, AI bias, and interoperability concerns must be addressed so that equitable, safe, and efficient deployment is enabled. The Metaverse healthcare future lies in the intersection of 5G, federated AI learning, quantum computing, and brain-computer interfaces (BCIs) that will make predictive healthcare, real-time medical simulation, and patient engagement progressively more streamlined. The article highlights the way convergence of AI, Digital Twins, XR, and the Metaverse is accelerating a paradigm shift towards proactive, predictive, and immersive healthcare ecology, the future of precision medicine.
