The evolution of healthcare systems towards value-based care has prompted the need for integrated approaches to personalized care. Digital health technologies (DHTs) hold promise to address challenges such as aging populations, rising healthcare costs, and disparities in access. However, ensuring DHTs are cost-effective, equitable, and responsibly implemented requires robust evaluation frameworks. Health technology assessment (HTA) is a multidisciplinary process for evaluating the value of technologies across their lifecycle—focusing on clinical, economic, ethical, and social aspects. The rapid development and unique features of DHTs challenge current HTA models, which were designed for an era when DHTs were not widespread. Incorporating real-world data and evidence into dynamic, iterative assessment models could yield timely and relevant insights into DHT value production. New frameworks must also address ethical, privacy, and cybersecurity challenges essential for building trust and safeguarding patient autonomy. This chapter explores the evolving role of HTA in the digital era, identifies gaps in existing frameworks across development, implementation, and post-market evaluation, and discusses strategies for methodological improvement and innovation. By fostering a more flexible, risk-based, and multidisciplinary approach, HTA can ensure DHTs are integrated safely and effectively into existing healthcare systems, aligning with the broader goal of delivering value-based, patient-centered healthcare.
