Levels of Mesopancreas Dissection in Robotic Pancreatoduodenectomy

The robotic approach to pancreatoduodenectomy (PD) has garnered substantial interest within the surgical community due to its precision, minimally invasive nature, and enhanced three-dimensional visualization. The advent of robotic surgery has revolutionized the way complex pancreatic procedures are performed, offering surgeons greater dexterity, stability, and a magnified view of intricate anatomical structures. This technological advancement is particularly beneficial in pancreatic surgery, where the anatomy is complex and the margin for error is minimal. Given the high risk of haemorrhage and the presence of several vascular structures with anatomical variations in the mesopancreas, robotic PD requires a profound knowledge of the anatomy and surgical techniques. The concept of mesopancreas dissection levels, introduced by Yosuke et al. in 2015, categorizes the extent of dissection required based on the type of pancreatic malignancy and associated vascular structures. This chapter provides a comprehensive guide to the three levels of mesopancreas dissection in robotic PD, emphasizing the importance of anatomical knowledge, surgical precision, and oncological principles.
