The pancreaticoduodenectomy (PD) is a challenging surgical procedure known for its time-consuming nature and technical complexity. Minimally invasive surgery (MIS) has gained popularity worldwide for its potential to ameliorate postoperative short-term postoperative outcomes (Ielpo et al., Updates Surg 71:137–144, 2019). However, the transition from open to laparoscopic surgery presents challenges for many surgeons, including the limited instrument range of movements, and the need to reverse instrument tip movement from surgical handmotion (Ielpo et al., G Chir 35(1–2):52–55, 2014). These difficulties hindered the widespread adoption of laparoscopic pancreaticoduodenectomy due to its technical complexity and the advanced skills required, especially for pancreatic reconstruction (Ielpo et al., Updates Surg 74:1479–1481, 2022). The introduction of robotic surgical systems has addressed these issues by providing enhanced visualization, precise instrument control, and tremor elimination, making complex procedures like pancreaticoduodenectomy more feasible and reproducible. In this chapter, we provide a stepwise report of the robotic PD technique developed in our center.
