The Role and Anatomy of the Mesopancreas in Pancreatoduodenectomy

The mesopancreas is a complex and debated anatomical structure critical to pancreatic cancer surgery, particularly during pancreatoduodenectomy. Described as a plate-like region of retropancreatic tissue, it includes areolar and adipose tissue, peripheral nerves, lymphatic vessels, and blood vessels. Situated posterior to the pancreatic head, it extends towards the superior mesenteric artery (SMA) and vein (SMV). While often referred to in surgical literature, its precise boundaries, composition, and classification as a true mesentery remain controversial. Alternative terms such as “retro-portal lamina,” “pancreatic head plexus II,” and “mesopancreatoduodenum” highlight the ongoing debate over its definition and oncological importance. Embryologically, the pancreas originates from ventral and dorsal buds, whose fusion forms intricate vascular and neural connections, further complicating the anatomy of the mesopancreas. This structure plays a pivotal role in cancer spread due to its rich lymphatic and neural networks. Surgical techniques for mesopancreas dissection vary based on tumor aggressiveness, ranging from limited resections for benign tumors to extensive excisions for malignancies. Achieving consensus on the mesopancreas’ anatomy and nomenclature is essential to standardize surgical approaches, improve oncological outcomes, and reduce inconsistencies in clinical practice. Multidisciplinary collaboration is vital to clarify its anatomical intricacies, enhancing precision in pancreatic cancer management.
