Robotic-Assisted Ileocystoplasty in Children

Robotic surgery represents one of the most significant technological advancements in modern surgical practice, notably for complex procedures such as the ileocystoplasty. Even though the open procedure remains the gold standard, an increasing number of institutions are developing minimally invasive techniques. For this study, the “W” configuration in open surgery has been adapted for the robotic technique, using Da Vinci Xi. The protocol includes a meticulous preoperative preparation, specific positioning of the patient and surgical team, the use of four robotic arms, and two experienced surgeons at the console. The technique has been performed on eight pediatric cases with complex underlying conditions and a history of prior abdominal surgeries. The mean operative time has been of 625 minutes. No cases of stenosis were observed, while an increase of 37.5% rate of leakage through the appendicular Mitrofanoff was found, compared to open surgery series. This experience demonstrates the feasibility of the robotic-assisted ileocystoplasty in children. The beneficial potential of this technique lies primarily in patients with a history of multiple previous abdominal surgeries. Technical improvements and increased experience are needed in order to optimize the results and reduce the complications.
