Patients with hypertrophic cardiomyopathy (HCM) frequently have arrhythmias, including atrial fibrillation (AF), supraventricular tachycardia (SVT), and ventricular tachycardia (VT)/ventricular fibrillation. While these arrhythmias can sometimes be asymptomatic, especially if of short duration, others can precipitate hemodynamic collapse, stroke, and sudden cardiac death (SCD). Therefore, risk stratification for management of arrhythmias, for both primary and secondary prevention, and for evaluation of their clinical significance in patients with HCM is of paramount importance. In this chapter, we will discuss the incidence, diagnosis, medical management, and role of diagnostic testing and ablation for arrhythmias in HCM patients.
