Perineal Crohn’s disease was described by Figg and Church in 2009 as a condition where the inflammatory process underlying Crohn’s disease infiltrates the tissues of the perineum [1]. It is clinically recognizable by its appearance (a waxy edema of the perineal skin, elephant-ear tags, painless fissures) and its behavior (non-healing wounds). The data in this important but basically ignored paper suggest that at least at the Cleveland Clinic (and therefore subject to referral bias) 19/72 (26%) of patients with perianal disease in the context of Crohn’s disease have Crohn’s-type inflammation in the perineum. The importance of recognizing its presence is discussed in this chapter.
