The ileal pouch-anal anastomosis is a way of restoring per anal defecation in patients who need their entire large intestine removed. It is commonly performed in patients with ulcerative colitis, but is also a frequent option chosen in patients with a severe form of familial adenomatous polyposis. One of the most common complications of an ileal pouch-anal anastomosis is pouchitis; inflammation of the ileal pouch. The incidence of pouchitis in patients with ulcerative colitis versus that in patients with familial adenomatous polyposis is a clue to the causes of pouchitis and underlies the question leading this chapter.
