Periodic application of topical fluorides such as fluoride varnishes and gels in clinical practice is an established mode of caries prevention on smooth surfaces of teeth. In recent times, the use of topical fluorides has extended to remineralize white spot lesions with high-intensity application schedules. This is further complemented by the emergence of Silver Diamine fluoride that arrests dentinal lesions in self-cleansing areas. On occlusal surfaces, where the effectiveness of topical fluorides is limited, pit and fissure sealants are recommended to avoid ingress of food particles in susceptible fissures leading to decay. The indications of sealants have expanded to include sealing non-cavitated carious lesions. This text looks at the changing treatment paradigms in preventive dentistry and suggests protocols that can help to effectively avail caries preventive effects of these materials.
