Pitfalls in the Management of the Rheumatic Patient as a Child

Juvenile rheumatic and musculoskeletal diseases such as juvenile idiopathic arthritis, paediatric systemic lupus erythematosus, juvenile dermatomyositis and vasculitis present unique diagnostic and management challenges. These conditions often differ from their adult counterparts in presentation and progression, and delayed or inadequate management can significantly impact growth, development and quality of life. Early symptoms like fever, fatigue and musculoskeletal pain may mimic common childhood ailments, leading to delayed diagnosis and inappropriate treatment. Furthermore, over-reliance on adult diagnostic criteria risks misclassification and missed early intervention. Chronic inflammation and prolonged corticosteroid use can impair growth, delay puberty and increase fracture risk, underscoring the importance of early disease control and corticosteroid-sparing strategies. The psychosocial impact, including anxiety, depression and social isolation, is frequently under-recognised, affecting adherence and overall well-being. Medication side effects and school absenteeism further compound challenges. Effective management requires early referral to paediatric rheumatologists, disease-specific diagnostic criteria, aggressive inflammation control and comprehensive support for psychological, educational and social needs.
