Hepatotropic and Non-hepatotropic Viral Hepatitis

Viral hepatitis remains a major global health problem. From the early days of identification of viral hepatitis A, followed by hepatitis B virus, and ultimately hepatitis C, other hepatotropic and non-hepatotropic causes of viral hepatitis have increasingly been recorded. The main hepatotropic viruses are numbered alphabetically from A to E with B, C, and D representing major causes of chronic liver diseases and causes of liver transplant referrals. These viruses are also risk factors for hepatocellular carcinoma (HCC). Hepatitis B, C, and D, while mostly causing chronic diseases, also have acute phases that could complicate biopsy assessment, especially when there is inadequate awareness of the natural history of these viruses. Viral hepatitis A and E are important causes of acute hepatitis and liver failure. The non-hepatotropic viruses, such as Epstein-Barr virus (EBV), adenovirus, cytomegalovirus, and herpes simplex virus, are mostly but not always opportunistic, causing major morbidity among immunocompromised hosts. Lastly, COVID-19 is a more recent entry with still unclear risk burden or impact. This chapter discusses these viruses from the pathologist’s point but emphasizes the critical role of understanding and incorporating the clinical, serological, and biochemical context in reviewing, working up, and reporting liver biopsies as part of overall viral hepatitis patient management.
