The LM stem contains a higher proportion of elastic tissue, which may explain the elastic recoil and high restenosis rates observed after balloon angioplasty in the past. Although bypass surgery has long been considered the gold standard treatment for LM stem disease, recent studies focusing on the safety and efficacy of PCI for ULM—considering significant advances in PCI techniques, stent technology, and adjunctive pharmacotherapy—have prompted a re-evaluation of PCI’s role in ULM disease. In this chapter, we examine the latest advances in LM PCI.
