The goal of adjuvant therapies, such as rTMS, in paresis treatment is to provide an optimally receptive neuronal environment in which natural recovery can happen and behavioral therapies can be integrated. This chapter discusses the current evidence and open questions on rTMS for the treatment of paresis after tumor resection in detail. We describe the existing evidence for the efficacy of low-frequency rTMS over the contralesional motor cortex to improve rehabilitation of postoperative paresis in the acute phase after tumor resection. Further, alternative treatment approaches as well as open questions in the field, such as patient stratification and treatment personalization, are discussed. Finally, we give an outlook on possible future developments, including brain-state-dependent and multilocus stimulation.
