An 86-year-old female patient with a history of systemic sarcoidosis, chronic renal failure, hypothyroidism, and dyslipidemia is admitted to the Cardiology Department for an episode of severe constrictive chest pain with sudden onset, not related to physical effort, irradiating to the left shoulder and left arm, accompanied by diaphoresis and palpitations that occurred 30 min prior to her presentation to the hospital. Her ECG recorded at admittance is presented in Fig. 1.
