The omental flap has been employed by reconstructive surgeons for many years and with a growing number of indications due to the unique healing properties inherent to the omentum itself. The immunologic, regenerative, and expansive tissue bed with dual blood supply make this an attractive donor site for vascularized omental lymph node transfer. This omental flap provides a solution during complex cases, even when the recipient site is compromised by infection, radiation, or otherwise. Two significant innovations that increased utilization of the omental flap are the advent of laparoscopic and subsequently robotic-assisted techniques for flap harvest. The ability to utilize a minimally invasive approach to access the omentum effectively eliminated one of the greatest deterrents for widespread adoption of the vascularized omental flap. In contrast to laparotomy, multi-port (MP) and single-port (SP) robotic omental harvest improve perioperative outcomes for patients. Importantly, as the field of lymphedema continues to expand in surgical treatment options and overall surgical efficacy, these patients have become excellent candidates for this procedure, which has been shown to be a highly effective and safe alternative to laparotomy.
