Twenty years after the first procedure, transcatheter aortic valve implantation (TAVI) has established itself as the most rapidly developing cardiac procedure in the twenty-first century. The road from a lifesaving procedure in inoperable or high-risk patients towards the treatment of choice for most patients with severe aortic stenosis is owed to continuous technological and technique advancements. TAVI surpassed surgery in the treatment of aortic stenosis not only by replacing it in many cases, but also by offering a cure for a whole new cohort of patients, previously constrained only to palliative medical treatment. This chapter aims to provide a comprehensive overview of TAVI in the management of heart failure in the context of aortic stenosis, highlighting past achievements, current practices in the context of established evidence, and possibilities for further development and impending concerns that need to be addressed in the future.
