Algorithm on Treatment in Fracture-Related Infections

Diagnosis of a fracture-related infection alters the course of rehabilitation of a patient. Their expectation of early discharge from the health care system is not fulfilled, and the socioeconomic cost of injury to the patient increases. Infection and fracture union need to be managed in the best possible manner. There are multiple points to be considered when managing such cases. The diagnosis has to be accurate after which, various treatment options can be considered, depending on the clinical setting and available resources. The treatment strategy should not place an unnecessary additional burden on the patient, but should also be good enough to ensure rapid control of infection and timely progression of the union. We outline a proposed algorithm to help clinicians decide on the treatment strategy by first understanding the host factors, then deciding on the future course of action. We briefly describe the host factors to be considered and the pros and cons of various treatment methods. This will help the clinician to analyze the clinical picture and justify their choices of treatment. The algorithm acts as a summary to guide the clinician to the best possible treatment strategy.
