Inflammatory bowel diseases (IBD) include ulcerative colitis (UC) and Crohn’s disease (CD) which are chronic inflammatory diseases affecting the gastrointestinal tract. Increased risk of thrombosis, despite the occurrence of gastrointestinal bleedings, is well established in IBD. Platelets are known for their role in hemostasis and also in the pathogenesis of thrombosis. Additionally, inflammation can lead to platelet activation, triggering their aggregation and clot formation without physiological needs leading to the formation of pathological thrombosis. In the context of IBD, platelets may also contribute to perpetuating the inflammatory phenotype by enhancing leukocyte recruitment and activation in the intestinal tissue. This suggests that platelets do not merely participate in hemostasis but also play a significant role in amplifying immune responses within the gut during IBD flares. This chapter aims to provide an overview of the significance of platelets in IBD, emphasizing their contribution to both extraintestinal thrombotic complications and the modulation of intestinal inflammation. Furthermore, it explores recent advancements in therapeutic strategies in IBD affecting platelets and sheds light on their potential as novel interventions in the management of IBD and associated complications.
