We present the case of a middle-aged man with a sedentary occupation but high levels of leisure-time physical activity. Following the implantation of an implantable cardioverter defibrillator (ICD) after surviving sudden cardiac arrest (SCA), the patient experienced a stable, uncomplicated recovery period. However, 3 years post-implantation, he suffered multiple episodes of electrical storms. This led to significant psychological distress, necessitating cardiac rehabilitation (CR) to address his general fear of shock delivery and specific reluctance to engage in exercise. In this chapter, we will discuss the key considerations for the rehabilitation of this patient, including a comparison of this case with expected outcomes and potential complications in CR programs, as guided by current scientific evidence.
